Forms & Releases Need Completion Before Start Of Hockey & Basketball Practices

With the anticipation of the start of high school hockey and basketball practices and the prospect of competition in mid-to-late January; high school athletes throughout the Kenai Peninsula Borough School District need to complete required forms and releases, insure a current physical, and contact coaches and athletic directors to guarantee everything is in order to hit the ice or take to the courts on day one of the revised sports seasons.

 

COVID-19 Hold Harmless, Release and Waiver of Liability

Statement from KPBSD Hold Harmless Form:  The Kenai Peninsula Borough School District (“KPBSD”) supports school activities such as, but not limited to, sports, music, art, drama, debate, and student government (referred to in this document as an “EVENT”). ). Allowing an EVENT during this time cannot be done without inherent risk to all participants.

Attached is the updated district COVID-19 waiver form. This must be signed and turned in on the first day of practice.

Student Health & Review Form

All athletes must have completed paperwork and a valid physical on file in order to participate.  Physicals are viable for 18 months.  Students with a physical dated to expire during a sports season must complete a new physical and have that form on-file before the expiration of the current physical.

Attached is the updated district Student Health waiver form. This must be completed, signed and turned in on the first day of practice.

Pre-Participation Athletic Form

Pre-Participation Athletic Forms are online with PlanetHS – ArbiterAthlete.  This digital platform will allow you, and your student(s), to complete and access athletic forms via computer, tablet, or mobile phone. It is HIPAA, COPPA and FERPA compliant.

Students/parents who participated in (or had a student participate in) an extracurricular activity during the 2019-20 school year have an active ArbiterAthlete account. Login and select the 2020-2021 school year and select school of eligibility.
